For patients with one or more of the following conditions we will invite you either by text or by letter to arrange an annual review appointment in or around the month of your birthday:-
Asthma, Atrial Fibrillation, Chronic Kidney Disease, Coronary Heart Disease, COPD, Diabetes, Heart Failure, Hypertension, Mental Health conditions, Learning Disabilities, Non-diabetic hyperglycaemia, Peripheral arterial disease, Stroke or TIA.
If you miss your review please ring us at any time to arrange this after your birthday month. We will usually send patients a second reminder a couple of months after their birthday. These reviews are important to ensure that blood monitoring, physical checks and medication reviews are done every year and, for many patients, mean that only one visit to the surgery is needed to ensure that everything is in place for the coming year.
