We have a team of social prescribers who we refer to as our Living Well Co-ordinators who are based in Hexham and can help patients with lots of non-clinical problems such as financial or legal matters, benefits advice, housing issues. In addition they can help patients who are experiencing loneliness, social isolation or those who might need some emotional support to get them through a difficult time.
